The Reaper Nurses are slim, female humanoids with light skin dressed in 20th-century nurse clothing. Their faces are visibly obscured by what seems to be a white paper bag with a medical "plus" sign marked on the front, though they can still see. Like the Matron, the Reaper Nurses wear red combs across the top of their bags. While it is unknown what their faces look like, some merchandise has shown that they wear pink lipstick and appear to have the mouth and chin of an average human. Their clothing comprises a skintight red and white rubber/latex dress, red buttons on the shirt and the skirt, white tights on their legs, and black first-aid signs featured on the shoulders and belt. They wear red gloves and red roller skates resembling heels.
